PyDicom installation fails w/o unfettered Net access

Description

PyDicom's setup.py relies on distribute and/or setuptools. On a computer where the packages it needs are not present, it will download them from PyPi.

However, this assumes unhindered access to the Net. If the computer in question isn't connected to the Net or is behind a proxy that requires authentication, PyDicom's installation will fail.

A couple of our users have already been bitten by this problem.

PyDicom's behavior isn't something we can change, but we should probably document it in the installation instructions.

Fixed on 3 Dec 2010 by adding a blurb on how to download & install distribute manually. It's annoying that distribute only provides a tarball for download. Opening that will be a real puzzle for Windows users.
